We’re excited for your family to join ours!

The checklist below outlines the items and documents you to register your child with Milestone Montessori.

For faster registration, download the forms below and fill them out before coming into Milestone. You can find other relevant documents, including the Parent Handbook, by visiting our Downloads page.

The below forms can also be filled out during registration at Milestone.

  • Enrollment Form
  • Parent Contract
  • Sleep Supervision Policy
  • Medication Package (if applicable)
  • $300 Registration Fee (cash or cheque. If cheque, please date it to day of registration)
  • First and 50% last month’s payment (cash or cheque. If cheque, please date it to day of registration)
  • Post-dated cheques for the remaining months (date to the 1st of each month)
  • Your child’s immunization records (or otherwise, a letter from your physician)
